This information will help us contact you in the case that your bicycle is stolen and recovered. Please NOTE: This is only the first step. After completing this form and submitting the information, you MUST present this information and as well as your BICYCLE (to verify serial number and apply decal) to an on-duty police officer or a Community Services Division staff member located at the University of Florida Police Department Community Services Division Building, within 30 days, to pick up your decal and verify your identity and bicycle information.
